Action LogPurposeTo record and provide a record of all internal actions including historical, current and proposals for each action. Derivation and Source RequirementsAll internal progress meetings including the Progress Meetings. CompositionFor all actions the Action Log will define: · A unique reference for all actions, · An owner for all actions, · The initial closure date, · Status (Open, Closed, Cancelled or Deferred), · The type of meeting where the action was raised (for example, safety review, progress meeting), · The expected (for open) or actual (for closed) closure date, · A clear and unambiguous description for each action, and · A summary of the progress to date including a date for the progress update. Quality CriteriaHas sufficient information been provided to ensure the action can be closed? Has an owner been identified? Are there plans and work packages in place to update the action log? Is the Action Log up to date? Quality Method and SkillsThe action log will be developed and maintained by the Project Support Office. Owners are responsible for providing updates and updating the Action Log. The open actions will be reviewed at the appropriate meetings. Issues LogA Project Issue is a generic term for any matter that has to be brought to the attention of the Project and requires an answer. A Project Issue may have a negative or positive impact on the Project. PurposeTo record and provide a record of all Project Issues including historical, current and proposals for each issue. Derivation and Source Requirements The Resource Reports, Progress Meetings and communications with the Project Management Team. Composition For all issues the following information is to be recorded: · Originator – who raised the issue, · Owner – who is responsible for addressing and closing the issue, · Date raised, · Unique reference number, · Title/description, · Priority, · Impact Analysis – scope of the issue, · Decision / Progress, · Proposed closure criteria, · Current Status, and · Date of closure. Quality CriteriaHas sufficient information been provided to assign an owner and address the issue? Is the owner the most appropriate person to address the issue? Are there plans and work packages in place to update the issue log? Is the Issue Log up to date? Has the closure criteria been defined and agreed? Quality Method and SkillsIssues can be raised stakeholders, by anybody working on the Project or anybody working in support of the Project. The Project Management Office is responsible for capturing and recording the issues. The Issue Owner is responsible for addressing the issue and updating the issue log. |
