Verification and Cross Reference IndexPurposeTo provide traceability from the requirements or defining documentation, into the Projects plans and specifications through to the acceptance and ‘sell of’ for all requirements. Derivation and Source RequirementsThe Verification and Cross Reference Index will be derived from the requirements documentation, the Projects plans, procedures, specifications, the design documentation, test specification and Phase Completion Reports. CompositionFor each requirement the VCRIs must include: · A unique reference. · Type of requirement, that is: Process, Safety, Technical, User (ATC), User (Asset Operator). · A unique reference(s) to where each requirement is addressed within the Project’s documentation. · The method used to prove compliance to each requirement, that is: Analysis, Demonstration, Inspection or Test. · The Project Phase where compliance will be demonstrated. · The unique reference to the results/documentation proving that the requirement was satisfied. A description of the source of the requirement and scope of the VCRI. For example, Project Plans or Technical Requirements. Quality CriteriaAre all the requirements addressed? Are all the items completed? Are the verification activities defined by appropriate plans? Has the VCRI been reviewed and had a sample audit completed? Are all the items identified above completed? Is the type of verification appropriate to the requirement? Quality Method and SkillsThe VCRI will be reviewed for completeness by ensuring that for all items defined by the above composition within the scope of the VCRI are complete. The reference to the results/documentation will be completed in the implementation phase. A sample audit will be conducted to demonstrate traceability. If errors and omissions are found then the scope of the audit will be increased. |
